Glen Perkins

Starter Glen Perkins, who took a no-hitter into the sixth in an Opening Day loss, gave up one run on four hits but left after 57 pitches in two innings (2.0 4 1 1 2 5) and did not return for the third.

Was this an injury or a pitch count removal?

Not an injury
I don't have the link on hand, but I read that it was more a mental thing and that he had labored so hard in two innings that the manager pulled him.

I'd like to check his pitch count for his first start.  I assume he has regained his control which appeared to disappear when he made it to AA and then reappear in AZ.  I wonder if AA Sox were fouling off a lot of pitches or if Perkins was just wild.

I believe this was a 1030 start, so maybe he's not a morning person?

a lot of organizations have a rule that if the pitcher throws more than 30 pitches in an inning they're done, or their game-total pitch limit comes down. i would guess that is what happened.

Discipline
Perkins was yanked for waving off his catcher when the catcher tried to come to the mound. It was a discipline issue.

Terry Ryan addressed the issue on KFAN radio
Friday afternoon.  He not only waived the catcher back but also berated teammates and coaches after the inning in the dugout when they were trying to calm him down.  I  heard that on the Chad Hartman show on KFAN

the problems with being a non-local fan...

It makes me wonder if the high pitch count was due to the catcher/manager/coach calling a bad game - resulting in Perkins waiving the calls off - or if he just didn't have anything that day and took his fustration out on his teammates in a very public manner.  The later would be very unfortunate and would make me reconsider Perkins ability to succeed as a ML'er.
