So far, it looks like the main problem with the internal part of the book is the missing Jason Kubel comment. I took a quick look and didn't see anyone else important who was missing, but I will review this in more detail when I get home. I'm confident that neither the editor nor the printer are responsible for this problem. I firmly remember writing the comment, so my guess is that a file got overwritten at the wrong time.
